The value of writing things down

A programme accumulates decisions. Very few of them are recorded, and almost none with the reason attached.\n\nTwo years later someone reverses a decision that was made for a good reason nobody wrote down. This is not a documentation problem. It is a memory problem that documentation happens to solve.
